Directions: Please print and complete the information as fully as possible. To be considered as an Upward Bound student, we must receive the following information:

a signed Upward Bound Application.
a copy of your high school transcript(ask your guidance counselor for a copy)
a signed income verification form (part of this packet) or proof of family income(such as income tax return)
